Data submission routes

There are a number of data submission routes:

1. Established event reporting systems

- For England based clinical departments connected to the Learning From Patient Safety Events (LFPSE), data will be extracted from LFPSE. To submit data, the trigger code PRIA24 and coding must be included in the incident description field on the department's local incident reporting system. For example, the following text would be included in the incident description on the local incident reporting system: PRIA24/L1/E1/MG/P7/3.2b/CF1b/CF4a.

- For Wales based clinical departments connected to Once for Wales (OfW) Concerns Management System, the coding is built into the incident reporting system and can be extracted by the OfW team. No further action is required to submit data.

2. Enforcing authorities

Regulators will have the opportunity to submit data directly to UKHSA for analyses.

3. Independent clinical providers

- Clinical departments in Northern Ireland, Scotland and the independent sector across the UK can submit data directly to the MEG. For further information, please contact MEG at:

Data must be anonymised by the clinical department prior to data submission. Incident and near miss event data should be submitted in either an Excel or CSV format.

Clinical providers are asked to include the following information for each incident or near miss event submitted to UKHSA:
  • Unique incident identifier (for example a Datix/Ulysses number)
  • Date the incident occurred
  • Date the incident was reported locally
  • Text description of event (ensure there is no personally identifiable information included)<
  • Completed coding taxonomy (for example PRIA24/L2/E1/MA/P1/3.5e/3.6b/CF3a)

  • The associated text description should include a brief account of the circumstances surrounding the incident or near miss, which could include:
  • How the event was detected
  • Number of patients affected
  • Implications for the patient(s)
  • Any corrective or preventative action taken

  • It is the responsibility of the clinical department to ensure data is anonymised prior to submission to MEG.
